I confirm, that at the time of booking the information provided is accurate to the best of my knowledge. I understand that all bookings are non-transferable or refundable. I agree to giving permission for Dewsbury Rangers coaching staff to administer First Aid treatment to my child should this be necessary. I can confirm that all food allergies and dietary requirements have been disclosed. I understand that Dewsbury Rangers Football Club and/or ProSport Group are not liable for any injury/accident as a result of my child taking part in the activities as advertised.
